安泰免費健康檢查附加條款第三條：
凡符合資格者，被保險人應於保單每屆滿三週年的應繳保險費到期日起一個月內，以書面向本公司提出申請，並依本公司指定的醫名單，於三個月內安排健檢查，但逾期未使用本檢查者，視為放棄。

上述條款是指，投保日起算的每滿3年，第4年起1個月內提出申請嗎？
目前已投保第11年度滿，剛繳12年度保費.....是現在提出，還是下年度提出呢？
另外，健康檢查的項目為何呢？
